Main responsibilities
You will audit medical record documentation to confirm correct coding and billing. You will check documentation using AMA CPT rules, ICD diagnosis codes, HCPCS codes and CMS billing policies. Your reviews will support many types of visits and services.
You will review outpatient encounters. You will audit diagnostic visits, emergency department records and minor procedures. You may also review homecare, hospice, nursing home and dialysis documentation.
You will look for errors in coding or documentation. You will explain the issue clearly so teams can understand what needs correction. Your work will support feedback and training for staff members who handle records and billing.
You will help strengthen coding quality. You will share findings with interested groups and support education that reduces errors. You may help teams understand proper code use and required documentation.
You will help maintain compliance with payer rules. You will apply knowledge of Medicare, Medicaid and commercial payer policies. You will help prevent billing mistakes that lead to denials or delays.
Education requirements
You must complete specific courses within one year of hire. These include CPT, ICD, HCPCS and medical terminology. Training must meet the department’s protocol and come from an approved source.
A background in a medical field is helpful. Preferred options include medical assistant programs, surgical assistant training, medical transcription or coding and billing programs. Courses in anatomy and physiology are also helpful.
Experience requirements
You must have experience working in healthcare, medical records or a medical business office. You must understand Medicare, Medicaid and commercial payer rules.

Certification requirements
