About the Role
The RN Care Manager serves as an essential part of the Care Management Team and works closely with patients, caregivers, physicians, social workers, payers, and other healthcare professionals. The role focuses on ensuring effective communication, proper utilization of resources, and smooth transitions of care.
The position requires strong clinical judgment, excellent communication skills, and the ability to collaborate across various disciplines. As part of the care continuum, the RN Care Manager supports patients in navigating their healthcare journeys while maintaining patient autonomy and addressing critical social factors affecting health outcomes. This full-time role includes 8-hour day shifts and participation in weekend rotations as required.
Responsibilities
- Coordinate and facilitate care across the healthcare continuum, supporting patients throughout their medical journey.
- Partner with interdisciplinary teams to ensure appropriate resource utilization and care planning.
- Support patients during transitions of care to promote continuity, safety, and high-quality outcomes.
- Participate in weekend rotations and contribute to a collaborative care management environment.
- Promote effective communication among care team members, patients, families, and external partners.
- Identify and address social determinants of health that influence patient outcomes.
- Maintain accurate documentation in compliance with regulatory and organizational standards.
Qualifications
Required:
- Bachelor’s Degree in Nursing or equivalent experience.
- Active California Registered Nurse License.
- Minimum 2 years’ experience in a healthcare-related setting (acute, ambulatory, post-acute, etc.).
- Minimum 2 years’ experience in Case Management, Care Coordination, or Utilization Management, or completion of the Care Manager Transitions in Practice (TIP) program.
- Recent inpatient RN experience with adult patients.
Preferred:
- Master’s Degree in Nursing or a related healthcare field.
- Case Management Certification (CCM or ACM).
- Experience in hospital-based Case Management or Discharge Planning.
